When attending your appointment with Dr Mark Moore, please bring any relevant pathology and/or imaging studies where possible.
If a referral is required, you can bring this on the day of consultation or alternatively it can be faxed to (08) 8239 2349. Procedures that are of a cosmetic nature do not require a referral.
If a surgical procedure is required, this will be fully explained at your appointment and you will be able to ask any questions you may have at this time. A date for surgery can be made at the appointment if desired.
